Your organization chart needs a top to down information, of who is the reporting manager to who. Below the name of the employee, at each level you can list down the department.

This is a very simple and easy thing. As you are in UAE I can understand how companies are working in UAE. You only need to check in your organization or company that how your company hierarchy is working. E.g: Start with descending order from Company owner Like Owner > Chairman / Managing Director > Director > Manager so on ...

I advice you to consider all the answers of all the above, and consider the following if you wish, - Mandate of the organization, vision, mission. - Organizational level, operational level, and CEO, no matter what is the volume of these levels, please address them. - Main operations, mandate of the positions not people to have the suitable linkages/ communications, to have the departments and address the decision levels/ responsibilities and authorities network, other things like SoPs, business card description for each position (this includes main features of qualifications, mandates, skills, training requirements, some how career path, etc. - Plans at all levels Startegic, operational must be addressed on the charts and match with it. - Other things will be concerning efficency, effectiveness, QA, Risk management, Stakeholders and partners, etc. -etc.
